Kelly Services, a global leader in staffing solutions, is seeking a Clinical Support Coordinator Associate for an industry-leading health insurance client in the Monterey Park, CA area! **Positions start on August 15, 2019 through December 31, 2019, with a potential to extend.**



**Job Description:**



Serves as initial point of contact for providers and members in the medical management process by telephone or correspondence. The Care Coordinator is responsible for coordinating resources relative to working to ensure members receive timely and appropriate diagnosis and treatment They work with members and providers on a case-by-case basis to achieve realistic outcomes. The primary goal of the Care Coordinator is to optimize member care in the ambulatory care setting to ensure appropriate treatment in consultation with the Board Certified Behavior Analyst, including performing regular reviews of provider treatment plans and the progress or lack thereof that a member is experiencing in consultation with the Board Certified Behavior Analyst. Care Coordinators will follow ALL cases in order to ensure appropriate treatment and quality of care, as well as, continuing medical necessity of the care the member is receiving since treatment can last from several months to a year or more, in consultation with the Board Certified Behavior Analyst .



**Responsibilities:**




+ Business Unit: Behavioral Health

+ Performs telephonic and written provider communication to support treatment of the member.

+ Ensures that approved treatment plans are used as developed and requests modifications to the treatment plan as appropriate, in consultation with the Board Certified Behavior Analyst .

+ Ensures the Provider via the treatment plan and the results is addressing both long and short-term goals, and regularly assesses the progress or lack thereof of the members progress, in consultation with the Board Certified Behavior Analyst.

+ Identification of barriers to meeting goals or complying with the plan. Development of a schedule for follow-up and communication with the provider.

+ Development of a schedule for follow-up and communication with the family and/or caregiver Process to assess progress against the case management plans for members, in consultation with the Board Certified Behavior Analyst BCBA .

+ Communicates collaboratively with all healthcare professionals involved in the members care to achieve expected outcomes .

+ Generates referrals to contracted ancillary service providers and community agencies in collaboration with the members provider.

+ Documents in the designated Case Management Database and authorization systems when applicable. Arranges and participates in multi-disciplinary patient care conferences. Monitor, document, and report pertinent clinical data as established per UM policy and procedure.

+ Generates referrals as appropriate for services.

+ Enters data in multiple systems as necessary for the generation of reports related to case management.

+ Conducts case review with the Case Manager, or subsequent superior, as appropriate. Performs other duties as may be assigned by direct supervisor or manager.



**Education/Requirements:**




+ Generally requires moderate experience in basic computer functions and software applications; MS Word and Excel

+ Minimum of 1-2 years experience

+ High School Diploma or GED Required; AA or BS/BA preferred

+ Bilingual in Spanish preferred

+ Excellent communication skills with individuals internal and external physicians, members, patients, and service providers

+ Knowledge of Medi-Cal and BHT services (especially, ABA treatment).



**Length of Contract:** August 15th through December 31, 2019



**Hours:** 8:30am-5pm



**Pay Rate** : $18.25 per hour
